Hunt and Hess Grade 1 for SAH
Asymptomatic or mild HA and slight nuchal rigidity.
Hunt and Hess Grade 2 for SAH
Moderate to severe HA, nuchal rigidity, no neurologic deficit other than CN palsy.
Hunt and Hess Grade 3 for SAH
Drowsiness, confusion, or mild focal deficit.
Hunt and Hess Grade 4 for SAH
Stupor, moderate to severe hemiparesis, possibly early decerebrate rigidity and vegetative disturbances.

Clinical implications of the Fisher scale
Greater Fisher grade correlates to increased risk of developing vasospasm.
Modified Fisher grade 0 for SAH
No SAH or IVH.
Modified Fisher grade 1 for SAH
Minimal/thin SAH with no IVH.
Modified Fisher grade 2 for SAH
Minimal/thin SAH with IVH.
Modified Fisher grade 3 for SAH
Thick SAH with no IVH.

Volume status in SIADH
Euvolemic or hypervolemic state.
Urine osmolarity and urine Na in SIADH patients.

FENa >1% for volume overloaded SIADH.
