see “alternate syndrome”: contra motor, ipsa cranial nerve findings
+/- ataxia
essential tremor
aka postural tremor
fine resting tremor, worse with intentional mvmt
better with alcohol
usually genetic
tx: BB, primidone (anticonvulsive agent)
parkinsons tremor
“pill rolling”
improves with intentional mvmt
intention tremor
fine with rest, zigzagging motion starts with pointing, or intending to touch nose, etc
indicates cerebellar dysfunction
Worst headache of my life, worse with sitting improves with laying down
meningitis (sitting causes hip flexion)
tx: ceftriaxone, vanco
add ampicillin if you suspect listeria
L hemiparesis and hemisensory loss What artery?
Also, what visual changes can you have?
MCA
R (contralateral) homonymous hemianopia
remember the optic radiation are in the parietal lobe
Central v. Peripheral Bell’s Palsy
Bells palsy = CN VII lesion
see: decreased lacrimation, hyperacusis, and loss of taste over the anterior 2/3 of the mouth, loss of nasolabial fold.
Central: forehead is spared, able to close the eye on the affected side
