why ultrasound for OB
A
- can confirm viability \+ fetal heart activity - dating \+ crown rump measurement (CRL) - number of embryos \+ twins, triplets, etc

importance of dating
A
- LMP used to calculate how many weeks and days a pregnancy is
- some menstrual cycles last longer than 28 days, others are shorter
- so we use first day of LMP because it is most accurate
- only certain testing done at specific times
- inaccurate dates lead to false positives or negatives

- nuchal translucency test
- maternal blood work

A
- tested before 14 weeks LMP
- measures amount of fluid behind baby’s neck ( over 3mm abnormal)

- all measurements and structures
- head, spine, abdomen, chest, femur, humerus, Anke, hand, placenta, umbilical chord, maternal uterus and cervix

28 weeks - 40 completed (41) weeks LMP
- monitering growth

- anatomy images not reassessed if normal
- monitoring for fetal well being (safe environment)
- measure for growth
- fetal heart rate
- biophysical profile

- doppler if needed
- vessel samples

- first trimester screen + Nuchal \+ PAPPA and hCG - 16 weeks \+ triple screen and quad screen - 24-28 weeks \+ glucose tolerance screen - gestational diabetes

- maternal weight, BP, urine
- uterine fundal height measurements
- position of baby
- fetal heart rate
- monthly at beginning - every 2 weeks if normal till 36 weeks - weekly if normal till term
- post dates ( overdue) require ultrasounds to check fetal well being and growth
