A 60 year old man presents with insidious, progressive weakness of the muscles of the body affecting the limbs, trunk, face and speech. The weakness is first noticed in the upper limbs. He has increased fatigue when exercising. He says he is clumsy, drops things and is tripping over a lot. He also has slightly slurred speech.
What is the likely diagnosis?

What are 4 signs of LMN lesions?
Muscle wasting
Reduced tone
Fasiculations
Reduced relfexes
What are 3 signs of UMN lesions?
Increased tone or spasticity
Brisk reflexes
Upgoing plantar response (positive babinski sign)
What are the supportive measures in MND? (2)
NIV
Riluzole (can slow progression of disease)
