Sensory ataxia, lancinating pains, neurogenic urinary incontinence, small irregular bilateral pupils poorly constricting to light (but ok with accommodation)

Tabes Dorsalis. Involves posterior column and nerve roots. Rx = IV penicillin for 10-14 days

Subacute fever, chills, malaise, HA, and dry cough. CXR shows mediastinal or hilar lymphadenopathy with focal, reticulonodular, or miliary infiltrates. Not TB. What’s Dx and Rx?
A
Histoplasma. Dx with histoplasma Ag of urine or blood. Biopsy shows granulomas with narrow based budding yeast. Most resolve spontaneously. Severe Dz gets PO itraconazole or IV liposomal amphotericin B.
