peripheral stromal ulceration that progresses circumferentially
autoimmune with no systemic complications

older- one eye
younger- both eyes and more aggressive

peripheral, mainly stromal, ulceration
Tx of Mooren’s Ulcer
corneal specialist
topical steroids
immunosuppressants + collagenase inhibitors
surgery
Peripheral Ulcerative Keratitis
PUK
associated with systemic disease
RA; Wegener’s granulomatosis
Signs of PUK
crescent shaped destructive lesions at juxtalimbal corneal stroma
neighboring scleritits
Tx of PUK
systemic immunosuppression
surgery by corneal specialist
Terrien’s Marginal Degeneration
idiopathic form of corneal peripheral thinning
slowly progressive
non-inflammatory- no infiltrates or redness
Symptoms of Terrien’s Marginal Degeneration
none
blur from astigmatism

peripheral gutter
superficial corneal vascularization, opacification, lipid deposits
Neurotrophic Keratopathy
epithelial breakdown and persistent ulceration that occurs with loss of trigeminal innervation
Signs of Neurotrophic Keratopathy
reduced corneal sensation
non-healing epithelial defects with thickened/ rolled edges
progressive stromal melt with minimal discomfort
Tx for Neurotrophic Keratopathy
discontinue all drops
anti-collagenase meds

Exposure Keratopathy
from incomplete lid closure
Signs of Exposure Keratopathy
dry eye symptoms
punctate epithelial changes that can progress to ulceration
Tx of Exposure Keratopathy
lubricants, lid taping, BCL

Thygeson’s SPK
idiopathic, bilateral
episodic
appearance of corneal opacities causing irritation and blur
Signs of Thygeson’s SPK
multiple small, elevated, gray epithelial disruptions across central cornea
Mx of Thygeson’s SPK
lubricants- mild cases
topical steroids
BCL
Recurrent Corneal Erosion Syndrome
epithelial erosions caused by poor attachment to underlying basement membrane
associated with prior trauma or corneal dystrophies
Symptoms of Recurrent Corneal Erosion Syndrome
significant pain
worse in morning
Signs of Recurrent Corneal Erosion Syndrome
epithelial defects
basement membrane irregularities- microcysts, swirls
Mx of Recurrent Corneal Erosion Syndrome
lubricants
antibiotics
combo of doxycycline and topical corticosteroid (prednisolone or fluorometholone)
Filamentary Keratopathy
mucus and cellular debris attach to epithelial surface
Causes of Filamentary Keratopathy
aqueous deficient dry eye, SLK, toxicity to drops
Tx of Filamentary Keratopathy
removal of filaments
acetylcysteine breaks up mucus strands
