Corneal Dystrophies Flashcards
General characteristics of corneal dystrophies
bilateral
genetic (AD)
not systemic
recurrent corneal erosions common
Signs of Epithelial Basement Membrane Dystrophy
EBMD
thickened basement membrane with deposits of fibrillary material between BM and Bowman’s layer
maps/ dots/ fingerprints
REE
Meesmann’s Epithelial Dystrophy
tiny epithelial cysts
Tx for EBMD
same as for Recurrent Epithelial Erosions
Tx of Meesmann’s Epithelial Dystrophy
lubricants
PTK
Reis-Bucklers’ Corneal Dystrophy
replacement of Bowman’s layer with CT bands
appears as subepithelial deposits that increase in density to form reticular pattern
Tx of Reis-Bucklers’ Corneal Dystrophy
laser keratectomy
keratoplasty
Lattice Corneal Dystrophy
amyloid plaques deposit in stroma and evolve to have a lattice appearance
Mx for Lattice Corneal Dystrophy
keratoplasty for opacification
Granular Corneal Dystrophy
hyaline deposits in stroma
resemble sugar granules that fuse causing corneal haze
Mx of Granular Corneal Dystrophy
keratoplasty for opacification
Macular Corneal Dystrophy
autosomal recessive
accumulation of glycosaminoglycans
poorly defined spots (macules) become hazy and affect full stroma
Mx of Macular Corneal Dystrophy
Penetrating keratoplasty
recurrence common
Schnyder (Crystalline) Corneal Dystrophy
disorder of lipid metabolism resulting in deposits of phospholids and cholesterol
Signs of Schnyder (Crystalline) Corneal Dystrophy
central haze- crystalline deposits
prominent corneal arcus
Tx of Schnyder (Crystalline) Corneal Dystrophy
laser keratectomy or keratoplasty
Fuchs’ Endothelial Dystrophy
accelerated endothelial cell loss
lack of cells results in edema
Symptoms of Fuchs’ Endothelial Dystrophy
gradually progressive blur, usually worse in morning
Signs of Fuchs’ Endothelial Dystrophy
guttata- beaten metal appearance of endothelial cells
Course of Fuchs’ Endothelial Dystrophy
progresses from stromal edema to epithelial edema to epthelial edema that can rupture
Mx of Fuchs’ Endothelial Dystrophy
Na Cl 5%, decrease IOP, hairdyer to dehydrate cornea
BCL, antibiotics, lubricants if epithelium ruptured
post lamellar or penetrating keratoplasty for scarring
Posterior Polymorphous Corneal Dystrophy
focal abnormal developmental differentiation of endothelial cells
subtle epithelial lesions
associated with glaucoma and peripheral anterior synechia
