- GI irritation
- nitrate reduced to nitrite by nitrate reductase
- nitrite causes oxidation of ferrous iron of Hb to ferric iron and formation of MetHb
- resp. insufficiency d/t inability of blood to carry O2
- fetal methemoglobinemia and death causing abortion
