When does assessment and data collection begin?
When the client enters the room.
Check their walk, eyes etc.
How often does a patient complete a health history questionnaire?
Once a year
What should be included on the health history?
Personal and social history
Chief complaint
Dental history/oral
Medical history
Conditions requiring medical clearance
For Premedication:
Underlying cardiac conditions
Prosthetic joint replacement
Artificial heart cakes
History of endocarditis
What on the health history form will need to be discussed further?
Anything circled YES
must write explanation near the YES in red
For bleeding gums and pain upon brushing on dental history form:
Teach disease process and prevention
For grinding/clenching teeth in dental history form:
Discuss night guard and prevention
For gum chewing and eating candy:
Discuss the caries process
For dry mouth in dental history:
Question about diabetes, vitamin deficiency, COPD, radiation, menopause etc
Prophylactic antibiotic premedication
Needed to fight bacteremia in the blood stream
Taken 1 hr before treatment
Clients with prosthetic total joint replacement may be susceptible, need medical clearance, supervising dentist will decide
May have to check with operating physician
What are the vital signs we take at each appointment?
Blood pressure
Pulse
Respiration
Temperature
Systolic
The peak or highest pressure
Result of ventricular CONTRACTIONS
Normal <120
Diastolic
Lowest pressure
