What is spine hypotensive syndrome?
a temporary, acute disorder due to the compression of the IVC by the weight of a pregnant uterus
What are risk factors for hypertensive disorders?
chronic hypertension
diabetes mellitus
multiple pregnancies
chronic renal dx
What’s chronic hypertension relating to pregnancy?
Bp of 140/90 or greater before 20wks gestation (w/o presence of hydatiform mole or hypertension) that persists for more than 42 days postpartum
What is the concern with chronic hypertension?
placental abruption
IUGR, hypoxia, fetal death, small/calcified placenta
What are the types of hypertensive disorders?
Chronic hypertension (3 types in PPT) Pregnancy induced hypertension preeclampsia eclampsia HELLP syndrome
What are some signs of an abnormal uterine artery Doppler signal?
early diastolic notch
reduced EDV resulting in high PI (>1.5)
high RI (>0.58)
RI above 95th percentile
Pregnancy induced hypertension usually develops when?
after 20 wks
may develop before with gestational trophoblastic dx
What are signs and symptoms of pregnancy induced hypertension?
BP proteinuria oliguria edema weight gain >3.6kg/week after 20wks headaches, visual disturbance
What is eclampsia?
the development of seizure or coma w/o an underlying neurologic or febrile origin (epilepsy or systemic infection) in a patient with preeclampsia
What does TORCH stand for?
toxoplasmosis others (syphilis, varicella, parovirus B19 infection, hep B) herpes CMV rubella
