Lymphoma Flashcards
what is the treatment regime for diffuse large B-cell lymphoma?
RCHOP
R: rituximab C: cyclophosphamide H: hydroxydaunomycin / daunorubicin O: oncovin / vincritine P prednisolone
what mutation is associated with burkitts lymphoma?
c-myc gene translocation , usually t(8:14)
what is characteristic of burkitts lymphoma on microscopy?
starry sky appearance
what is a complication of treatment for burkits lymphoma and how is this prevented?
tumour lysis syndrome due to rapid response to chemotherapy
give rasburicase before starting chemotherapy
rasburicase is a recombinant form of urate oxidase, an enzyme which catalyses the conversion uric acid to allopurinol (allantoin is more water soluble than uric acid so renal excretion is better)
what are features on presentation which distinguish hodgkins from non-hodgkins lymphoma?
hodkins lymphoma can have alcohol induced pain at lymph node
B symptoms occur earlier in hodgkins compared to non-hodgkins
extra-nodal disease much more common in non-hodgkins
how is lymphoma staged?
Ann Arbor staging
I: single lymph node above diaphragm
II: 2 or more lymph nodes above diaphragm
III: lymph nodes on both sides of diaphragm
IV: + anaemia / ITP
A: no systemic symptoms
B: fever > 38, weight loss > 10% in past 6 months, night sweats
what are poor prognostic indicators for non-hodgkins lymphoma?
B symptoms stage IV age > 45yrs male albumin < 40g/l WBC > 15000 /ul lymphocyte count < 600 or < 8% haemoglobin < 10.5
what type of anaemia is found in hodgkins lymphoma?
normocytic anaemia
Reed sternberg cells are associated with what type of lymphoma?
hogdkins lymphoma
‘stary sky’ appearance is associated with what type of lymphoma?
burkitts lymphoma
what are the histological classifications of hodgkins lymphoma?
nodular sclerosing - more common in women, african subtyp
mixed cellularity - associated with large number of reed-sternberg cells
lymphocyte predominant - best prognosis
lymphocyte deplete - worst prognosis
