Lymphocytosis Flashcards
definition of lymphocytosis
is defined as elevation of the absolute lymphocyte count for the patients age. Absolute lymphocyte count is >3.5 x 10^9/l means lymphocytosis
most frequent causes of lymphocytosis
- infectious (EBV, influenza, CMV, HIV, TB, toxoplasmosis)
- non-infectious (due to hypersensitivity, autoimmune, smoking, stress, vaccines)
- neoplastic (B or T cell origin)
clinical meaning of lymphocytosis
- increased LDH (>248)
- splenomegaly
- lymphadenopathy
lymphocytosis with reduced platelets and increased lymphocytes in a young patient means
acute lymphoid leukemia, on a blood smear you see atypical activated lymphocytes
list B cell derived lymphocytosis
CLL, MBL, burkitts lymphoma, follicular lymphoma*, pro-lymphocytic leukemia, mantle cell lymphoma (associated with high-level lymphocytisis *)
list T cell derived lymphocytosis
sezary syndrome, large granular lymphocyte leukemia, T cell non Hodgkin lymphomas, T cell pro-lymphocytic leukemia
list T cell derived lymphocytosis
sezary syndrome, large granular lymphocyte leukemia, T cell non Hodgkin lymphomas, T cell pro-lymphocytic leukemia
what is DRESS syndrome
Leukocytosis associated with drug hypersensitivity. Involves rash fever, lymphadenopathy, lymphocytosis, hepatitis, involvement of lung, kidney, heart or pancreas.

you test CRP with
pro calcitonin
if there are heterogenous pleomorphic lymphocytes suspect
reactive lymphocytosis
if there are homogenous lymphocytes suspect
lymphoma or neoplastic conditions
lymphocytes of chronic lymphocytic leukemia show as
small lymphocytes with a little cytoplasm ring, and a big reactive nucleus. They have gumprecht shadows or smudge cells (due to high vimentin)
