Define chronic illness
a condition that lasts a year or more and requires ongoing medical attention, limits activities of daily living, or both
What are some nonfatal chronic illness?
conditions such as osteoarthritis or hearing or vision problems
What are examples of serious, eventually fatal chronic conditions?
cancers, organ system failures, dementia and strokes
Define frailty
a fatal, chronic condition in which the body has few reserves left, and any disturbance can cause multiple health conditions and costs
What is Frailty Syndrome?
-an independent geriatric syndrome that may be seen in older adults with multiple comorbidities; becoming more common with the increased longevity
-includes both physical and mental decline
-leads to an increased risk for morbidity and mortality
What are the signs and symptoms of frailty syndrome?
weight loss, fatigue, muscle weakness, slow or unsteady gait, declines in activity
-use the clinical frailty scale to assess
What are some ways of preventing chronic illness?
-lifestyle choices
-early detection
-management of risk factors (improved diet, exercise, medical treatment, smoking cessation if applicable)
-promotion of cognitive health

Describe the Shifting Perspectives Model?
-views living with chronic illness as an ongoing, continually shifting process in which the person moves between the perspectives of wellness in the foreground and illness in the foreground
-sees the patient as a partner in care rather than a client
-illness is a part of who the person is but doesn’t define the person
