Causes of primary lipidaemia
monogenic such as familial hypercholesterolaemia but mostly polygenic causes
Causes of secondary hyperlipidaemia
Alcoholism, hypothyroidism, diabetes, diet
Examples of statins
Simvastatin, pravastatin, atorvastatin
How do statins work
Inhibit HMG-CoA reductase in liver, helps clear plasma LDL cholesterol, decreases TGs and increases HDLs
What are statins used for
First line for hypercholesterolaemia to reduce progression of atherosclerosis, primary prevention of MI and stroke
Secondary roles of statins
Reduce vascular inflammation and thrombosis, and plaque stability
Examples of fibrates
Clofibrate, fenofibrate, gemfibozil
Action of fibrates
Inhibit VLDL release and accelerate lipoprotein lipase in tissue capillary, by activating peroxisome proliferator activator receptor alpha.
Overall effects of fibrates
Reduces plasma triglycerides, LDLs and increases HDLs
What are fibrates used for
First line for high TG (pancreatitis), variable effects on CAD, can be combined with statins
Side effects of fibrates
Myositis, flu and gallstones
Action of nicotinic acid (Niacin)
Inhibits diaglycerol acyltransferase, reducing TG synthesis and release from liver, lowers plasma TG and LDLs
Used of niacin
Combined with fibrates for hypertriglyceridaemia
Side effects of niacin
Poorly tolerated, flushing, itching, dizziness, palpitations, gout
