Cause of secondary hypertension
Renal disease, obesity, pregnancy, endocrine (aldosteronism)
Complications of hypertension
Ischaemic heart disease, cerebrovascular incident, hypertensive retinopathy, hypertensive nephropathy, heart failure
Reading for stage 1 hypertension
> 140/90 clinic, >135/85 home
Readings for stage 2 hypertension
> 160/100 clinic, >150/95
Readings for stage 3 hypertension
> 180/120 clinic
Tests for new diagnosis of HTN
Urine albumin:creatinine ratio, HbA1c, renal function, lipids, fundus examination and ECG
Medication control for HTN (ABCD)
ACEI, beta blocker, calcium channel blocker, thiazide like diuretic
Another option in place of ACEI
ARB such as candesartan
Eg of ACEI
Ramipril 1.25mg up to 10mg
Eg of beta blocker
Bisoprolol 5mg up to 20mg
Eg of calcium channel blocker
Amlodipine 5mg up to 10mg
Eg of thiazide like diuretic
Indapmide 2.5mg
What changes to medication need to be made if a patient is African or Afro-Caribbean descent
Use ARB instead of ACEI
What drug to add if potassium is less than or equal to 4.5mmol/L
Consider a potassium sparing diuretic such as spironolactone
What drug to add if potassium is more than 4.5mmol/L
Consider an alpha blocker such as doxazosin, or a beta blocker such as atenolol
