Indications to start pharmacological management
Stage 1 HTN <80 years with end organ damage, CVS, renal disease, diabetes or 10year CVS risk >10%.
Anyone with stage 2 HTN
Step 1 of pharmacological management
ACEI if <55 years
Calcium channel blocker if >55 years or African or Caribbean
Alternative for step 1 management
ARB
Step 2 of phamacological management
Combine CCB and ACEI or ARB
Step 3 of pharmacological management
Add thiazide like diuretic - Indapamide
Step 4 of pharmacological management if potassium is >4.5mmol
Increase thiazide like diureti or,
Alpha blocker or,
Beta blocker
Pharmacological management if potassium is <4.5mmol/L
Add spironolactone
Eg of ACEI
Ramipril, catopril, enalapril, lisinopril
Eg of ARB
Candesartan, losartan, valsartan
Eg of calcium channel blocker
Nifedipine, amlodipine, verapamil, diltiazem
Eg of thiazide like diuretic
Indapamide
Eg of alpha blocker
Doxacosin
