Therapeutic Classification
Anticoagulant
Pharmacologic Classification
Direct thrombin inhibitor
Mechanism of Action
Directly inhibits action of thrombin without requiring intermediate step of antithrombin III inhibition. No specific antidote for lepirudin overdose.
Indications
HIT (heparin induced thrombocytopenia)
Contraindications
- Active bleeding
- aPTT ratio >2.5
Adverse Effects
- *Abnormal bleeding
- Bleeding from puncture/wound site
- Epistaxis (nosebleed)
- Hematuria
- Vaginal bleeding
- GI bleeding
- Hypersensitivity (including anaphylaxis)
Drug interactions with oral anticoagulants?
Increased effect
Drug interactions with drugs that inhibit platelet aggregation? (e.g. aspirin, indomethacin, and ibuprofen)
Bleeding
Drug interactions with ginger, garlic, green tea, feverfew, and gingko?
Increased risk of bleeding
pregnancy category?
Pregnancy Category B
Nursing Considerations
- Complete health history
- Baseline blood tests
- Monitor aPTT
- Collect blood for testing 30 minutes prior to dose
- Monitor for signs of bleeding
- Do not give oral anticoagulants
Patient & Family Teaching
- Immediately report unusual bleeding
- Avoid activities that may result in injury
- Do not take other drugs or supplements
- Immediately report suspected pregnancy
- Do not breastfeed
