Mechanism of Action
A
Occupies binding sites on plasminogen and plasmin. Prevents digestion of fibrin clot by plasmin.

A
- Aplastic anemia
- Hepatic cirrhosis
- Postoperative cardiac surgery
- Certain carcinomas
- Hemophilia A

Contraindications
A
- History of CVA in last 3 months
- Recent trauma/surgery
- Active bleeding
- Severe HTN
- Intracranial neoplasm
- AV malformation

Nursing Considerations
A
- Complete health history
- Check IV site frequently for extravasation
- Observe for thrombophlebitis
- Assess for signs of myopathy
- Monitor vital signs and urine output

- Immediately report adverse effects
- Make position changes slowly
- Observe for increased toxicity with oral contraceptives and estrogen
- Immediately notify if pregnancy suspected
- Do not breastfeed
