Mechanism of Action
A
Partial agonist at mu receptors, antagonist at kappa receptors.

Indications
A
- Relief of moderate to severe pain → parenteral route
- Treatment for opiod withdrawal and dependence → sublingual route

Contraindications
A
- Respiratory depression
- Cardiac arrhythmias

Adverse Effects
A
- Respiratory depression
- Prolonged QT interval with long term use of patch form

Nursing Considerations
A
- Protect patch site from heat (may release more)
- Monitor EKG
- May take 1 -2 weeks to become effective
