What are the 8 elements required to be on a Rx label?
The eight required elements that must be typed on a
label are the following:
• Name and address of the dispenser or pharmacy
• Serial number of the prescription order (referred to as
the prescription number)
• Date that the prescription was filled
• Name of the prescriber (the doctor)
• Name of the patient
• Directions for the patient on how to take or apply the
drug, and any precautionary statements such as “Do
not drink alcohol when taking this drug.”
• Number of refills for this medication
• Name, strength, and how much of the medication
What is the identification number used for on a label?
To identify the prescription to the patient. In some cases
there may be a format that identifies the specific drugstore
of a chain, the pharmacist, and the patient for
which the prescription was filled.
What do the abbreviations q.a.m., q.p.m., and q.i.d. mean?
q. a.m. means every morning, q.p.m. every night, and
q. i.d. four times a day.
Why isn’t it good practice to type abbreviations in the patient directions on a label?
The patient may not understand the abbreviation and
could take the wrong dosage
