KOH exam steps
A
- use to rule in or rule out tinea
- use for skin infection, nail infection better use culture (take as much as possible as proximal as possible)
- thin and small sample
- scrape with one glass slide, collect with other glass slide
- do not use currette or blade
- apply 1 drop 10-20% KOH with DMSO to slide
- apply drop chlorozole black E - stains chitin in fungal wall bluish black
- cover slip
- flame underneath for 5 sec, wipe off carbon residue
- scan on 10 power, 40 power to identify
- looking for something same thickness as cell membrane, but a little out of focus with skin layers
- presence of 1 hyphae in one sample = diagnosis

Hyfercation technique
A
- physical spark occurs before contact with skin
- use hi setting, start at 30
- wear gloves
- local anesthetic with epinephrine to desired area
- area will turn black and charred or white, lifting up area in epidermal bubble
- curette or tissue clipper the tissue off to level of superficial fascia
- cauterize base of lesion to stop bleeding and kill any remaining tissue (turn down the dial to 15)
- don’t go deeper than an 1/8 in generally

liquid nitrogen treatment
A
- remove keratin layer above lesion first with scalpel blade
- pour liquid nitrogen into a cup with coozie
- ideally 3 for 30 seconds or patient tolerance
- dip application tip into liquid nitrogen applying pressure and twisting slightly
- skin will turn white
- put bandage/gauze on area
- will likely blister after tx, used bandaid and anitbiotic
