What range of KD?
3 months - 4 years
What is the typical presentation?
3 y/o boy presents with fever, polymorphic rash, conjunctivitis and strawberry tongue. O/e he is noted to have unilateral cervical lymphadenopathy
What diagnostic criteria is used for KD?
AHA criteria
What is the AHA criteria for KD?
5 days of fever that improves with antibiotics + 4 of the following
- Bilateral conjunctivitis
- Polymorphous rash
- 1 skin finding
- > Erythematous soles
- > hand/feet oedema
- > unilateral cervical lymphadenopathy
- 1 mucous membrane finding
- > fissured lips
- > strawberry tongue
Investigations of KD?
Bloods
CxR
Coronary angiogram
Blood results?
ESR raised Anaemia Thrombocytosis High WCC LFTs raised
What might be shown on coronary angiogram?
Coronary artery aneurysms
What can coronary angiogram be used to predict in KD and how is it calculated?
Risk of myocardial ischaemia
Low risk = no CAAs
Med risk = regressed CAAs
High risk = large CAAs
1st line treatment?
IV immunoglobulin
2nd line?
Prednisolone
3rd line?
Infliximab
What can be used to treat the conjunctivitis?
Chloramphenicol
What must be given in conjunction with all treatment of KD?
Aspirin HIGH DOSE
