Main finding of the study:
Administration of Carprofen or the use of nerve blocks with bupivicaine improved analgesia after periodontal treatment more than did butorphanol, morphine or nerve blocks with lidocaine.
How do local anesthetics work?
Local anesthetics block the formation and conduction of a nervous stimulus from the affected area. They block influx of the sodium channels of nociceptive fibers preventing generation and propagation of an action potential to the spinal cord along the nerve.
What is the onset of action, duration and maximal dose of lidocaine 2% for dogs?
Onset: 1-5 mins
Duration: 1-2hrs
Max dose in dogs: 4-6mg/kg
What is the onset of action, duration and maximal dose of bupivacaine 0.5% for dogs?
Onset: 5-10min
Duration: 2-4hr (other studies show some have >24hrs)
Max Dose in dogs: 2mg/kg
How does carprofen work?
Carprofen is an NSAID. It inhibits cyclooxygenase 1 (COX-1) and 2 (COX-2) enzymes from breaking down arachidonic acid and producing prostaglandins, which are proinflammatory compounds. These play a major role in perpetuation of inflam and sensitize neurons to chemical, mechanical and thermal stimuli. NSAIDs suppress pain transmission both peripherally and centrally.
What did periodontal treatment in the study include?
Hand and ultrasonic scaling, subgingival scaling with Gracey curette.
Patients requiring further tx for excluded.
How and when were study subjects assessed?
Approx 30 min after recovery & atipamazole administration. Visual analog scale and Modified Univ. of Melbourne Pain Score were used to assess pain.
