What is ventricular fibrillation caused by?
A
- chronically occluded coronary artery causing a scar and an electrical reantrant circuit to be set up
- unstable plaque leading to ischaemia it woud have to happen in an electrically important area of the heart
- ischaemic cardiomyopathy will allow many re-entrant circuits

What is the presentation of MI?
A
o severe, associated autonomic upset (sweating, fear, breathlessness), angor animi (sense of impending doom)
o ongoing pain despite >10mg morphine

What is the presentation of PE?
A
o breathlessness, dull, maybe pleuritic

What is the presentation of aortic dissection?
A
o tearing, excruciating, severe then eases

What can relieve symptoms in MI?
A
Revascularisation
o CABG, PCI

What are the drugs used in IHD?
A
Aspirin -Antiplatelet ß blockers -Slow heart rate, reduce O2 demand Statin -Reduces cholesterol ACE inhibitor -Reduces blood pressure
