What is ventricular fibrillation caused by?

A
  • chronically occluded coronary artery causing a scar and an electrical reantrant circuit to be set up
  • unstable plaque leading to ischaemia it woud have to happen in an electrically important area of the heart
  • ischaemic cardiomyopathy will allow many re-entrant circuits

A

o severe, associated autonomic upset (sweating, fear, breathlessness), angor animi (sense of impending doom)
o ongoing pain despite >10mg morphine
