What is IBS?
chronic syndrome (more than 3 monts) what is characterized by abdominal pain or discomfort that occurs in association with altered bowel habits.
what plays a large role in IBS?
mental health
symptoms of IBS
beings in late teens to twenties
abnormal stool frequency
HALLMARK- abdominal discomfort that is relieved immediately after defecation.
Other somatic or psychological complaints (e.g. dyspepsia, chest pain, headaches, fatigue, myalgias, anxiety, depression)
What is necessary for the diagnosis of IBS?
abdominal discomfort or pain with at least 2-3 of the three features:
relieved with defecation
change in frequency of stool
associated with a change in stool form
3 major categories based on predominant bowel habits of IBS:
IBSC or IBSD
IBS-U (infrequent)
IBS with mix of constipation or diarrhea- IBSM
What does it mean that IBS is a diagnosis of exclusion?
carefully rule out other disorders with a detailed history, physical exam, labs/imaging, or some combination before considering IBS.
colonoscopy or simoidoscopy for IBS patients are not recommended for young patients without alarming SX. What age and criteria are there for performing one?
young patient with no improvement of conservative management.
over 50 with no previous evaluation to rule out malignancy
What is the definitive treatment for IBS?
no definitive treatment currently exists.
Diet: FODMAP, slowly introducing other foods, avoiding trigger foods.
No allergy testing is required
treat SX: antispasmodic agents, anti diarrhea, anti constipation and psychotropic medications.
