pill-induced, eosinophilic, and infectious
what are some medications that cause direct esophageal injury?
antibiotics- tetracycline, doxy, clinda
anti-inflammatory- aspirin, NSAIDS
Bisphosphonates- (meds that help with bone loss)
signs and sx of esophagitis
medication induced pain may present with retrosternal pain or hear burn, odynophagia, and dysphagia
pill induced may have hematemesis, abdominal pain, weight loss.
w candida esophagitis the hallmark is odynophagia, localized to discrete retrosternal area. Common in HIV patients and patients with hematologic malignancies.
labs for esophagitis
in infection is considered:
CBC
Specimen culture
Swab for candida
Consider endoscopy
treatment for esophagitis
treat the underlying cause: stop taking offensive medication and take pills with water
evaluate for immunocompromised conditions if candida esophagitis
INITIAL CARE: broad spectrum antibiotics in patients who present with fever and WBC
