A

major bleeding:
stop warfarin
give IV vit K

minor bleeding >8
stop warfarin 
give IV vit K
repeat if still high after 24hrs
start warfarin when <5
no bleeding > 8
stop warfarin 
give oral vit K
repeat if still high after 24hrs
start warfarin when <5

minor bleeding INR 5-8
stop warfarin
give IV vit K
start warfarin when <5

no bleeding INR 5-8
reduce number of doses of warfarin
