Heart Failure Flashcards
what is heart failure
the heart fails to pump blood
types of heart failure
low output
high output
what consists of low output heart failure
reduced left ventricular ejection fraction
preserved left ventricular ejection fraction
what are causes of reduced left ventricular ejection fraction
ischaemic heart disease
dilated cardiomyopathy
myocarditis
arrhythmia
what are causes of preserved left ventricular ejection fraction
hypertrophic obstructive
restrictive
cardiac tamponade
pericarditis
what is systolic dysfunction related to
reduced left ventricular ejection fraction
what is diastolic dysfunction related to
preserved left ventricular ejection fraction
right side heart failure symptoms
build up of blood behind heart
signs: peripheral oedema hepatomegaly bloating anorexia nausea
left side heart failure symptoms
diastolic or systolic problem in the ventricles
signs: dyspnoea nocturnal dyspniea orthopnoea pulmonary congestion bi-basal crackles cyanosis, CAP refill, pale looking
what consists of high output heart failure
normal on echocardiogram and normal ejection fraction
causes of high output heart failure
anaemia pregnancy thyrotoxicosis pagets arteriovenous malformation
investigations
BNP levels
echocardiogram
ECG
chest x-ray
what would be seen on the x-ray
alveolar oedema Kerley B line cardiomegaly dilated upper vessels pleural effusion
chronic management
ACEI/beta blocker
spironolactone
ivabradine - >75bpm/LVF < 35%
valsartan - <35 % and symptomatic on ACEI
hydralazine/nitrate - can’t tolerate spironolactone and om treatment for left ventricular systolic dysfunction
digoxin
cardiac resynchronisation - <30% and widened QRS
defib
acute management
ABCDE
loop diuretics
Resp failure:
CPAP
Hypotension:
inotropes
vasodilator
