signs
usually uni-lateral intense pain swelling varicose veins redness pitting oedema
Well’s score
assesses the likely risk of a DVT
calf swelling >3cm recent surgery or bedridden for >3 days pitting oedema active cancer leg swelling superficial veins previous DVT localised tenderness along venous system paralysis/plaster of lower legs unknown cause
investigations
D-dimer - sensitive but not specific
Dopple USS
Management
DOAC
LMWH/warfarin
give therapy if having to wait for doppler US
3 months - if reversible cause
6 months - non-reversible/continuous DVT occurring/un known cause or active cancer
complications of DVT
pulmonary embolism
travels up the right side and lodges it self in the lungs
well’s score and investigations protocol
well’s score - 1 - D-dimer
well’s score>2 - doppler USS within in 4hrs
what must be done if doppler USS can’t be done within 4hrs
give DOAC
when should LMWH be given
if there is renal impairment
antiphospholipid syndrome
