Live vaccines and Ig products
Delay (3-11 months) based on product
Redose vaccine if Ig given w/in 14 days of vaccination
Contraindications: Rotavirus
SCID, h/o intussusception
Contraindications: Pertussis
Severe encephalopathy w/in 7d of previous dose
Contraindications: IPV
Severe allergic reaction to streptomycin, polymyxin B, neomycin
Contraindications: Hep A
Hypersensitivity to alum of phenoxyethanol

Post-exposure prophylaxis: Hep B+ mother
Hep B Ig and HBV w/in 12 hours of birth (different sites). HBV at 2 and 8 months. Test at 15 months
Post-exposure prophylaxis: Hep B unknown
HBV w/in 12 hours of birth. Test mom!
Indication for Tetanus Ig
Contaminated wound with incomplete or unknown immunization status
Post-wound tetanus prophylaxis
Dirty wound > 5 years
Clean wound > 10 years
Diptheria formulations
Little ādā - lower immunogenicity, less local rxn
Diptheria vaccine by age
< 7 yo = DTaP
8-11 yo = Td
> 11 yo = Tdap
Pertussis efficacy
70-85% (acellular)
Pertussis: adverse effects
Local irritation, inconsolable crying, seizures (usually febrile), hypotonic episodes, limb swelling
OPV risk/benefit
VAPP/heard immunity
PCV-13 conjugate
Diptheria toxin
PPSV23 indications
Children > 2 yo w/ SCD, asplenia, HIV, CSF leaks, chronic illness. 2 weeks prior to splenectomy, chemo, cochlear implant
Influenza vaccine schedule
Yearly (2 doses needed in 1st season if < 8 yo). Live vaccine should be given 4 weeks later if not given on same day of other live vaccines
Inactivated flu vaccine (TIV) side effects
Oculo-respiratory syndrome, GBS
Contraindications: live-attenuated flu vaccine (LIAV)
< 2 yo, egg allergy (TIV = OK!), h/o GBS, asthma, recurrent wheezing
Varicella efficacy
95% for severe disease (70-85% for mild)
Varicella vaccine associated rash
6-10%, transmission possible
Measles schedule
12 mo, 4-5 yo (99% effective w/ 2 dose)
Can be given if > 6 mo during outbreak (must redose at 12 mo)
HPV vaccine serotypes
16/18 - cervical cancer
6/11 - warts
Meningococcal vaccine serotypes
A, C, Y, W135 (no B!)
