Hyperosmolar Hyperglycaemic State (HHS) Flashcards
1
Q
What is DKA most common?
A
T1
2
Q
What is HHS most common?
A
T2
3
Q
What is the patho of HHS?
A
Hyperglycaemis but no ketonaemia as still some insulin to supress ketogenesis and dehydration causes increased osmolarity
4
Q
What is the cause of HHS?
A
- Initial presentation
- Infection/other acute illness
- Non adherence to diabetes med
5
Q
What are symptoms and signs of HHS?
A
- Collapse/confusion
- Dehydrated
- Kussmaul breathing
- NO ABDO PAIN (DKA)
- Nausea
- Vomiting
6
Q
What is ketone level in HHS?
A
<3mmol/L
7
Q
What is pH in HHS?
A
normal (7.35-7.45)
8
Q
What is plasma glucose is HHS?
A
> 30
9
Q
What is the management plan of HHS?
A
- Fluids started (normal saline) + KCL (if K+<5.5)
- Iv insulin after fluids only when K+ not <3.5
- Include dextrose in fluid if it falls <14
- Treat underlying cause e.g. Abx
10
Q
What does insulin do?
A
drives potassium into cells
