Is diabetic neuropathy common?
A
- most common chronic complication of diabetes
- diagnosed after exclusion of other causes
- 50% of people with diabetes will develop DN during their lifetime

What are RF for diabetic neuropathy?
A
- poorly controlled hyperglycaemia
- older age (e.g., >70 years)
- prolonged duration of diabetes (e.g., >10 years)
- tall stature
- In T2DM: cardiovascular too

What are symptoms and signs of diabetic neuropathy?
A
- Some asymptomatic
- Pain (peripheral)
- Loss of sensation (peripheral) - stocking distribution
- Dysaesthesia (peripheral)
- Painless injuries (peripheral)
- Reduced or absent ankle reflexes
- Erectile dysfunction

A
- Uraemia
- Hypothyroidism
- Chronic high alcohol intake
- Drug-induced neuropathy
- Heavy metal poisoning
- Guillain Barre syndrome
- Myasthenia gravis

What investigations do you carry out for diabetic neuropathy?
A
- Clinical diagnosis
- Fasting blood glucose + HbA1c
- Serum TSH
- Serum Vit B12
- Electrolytes ,urea, creatinine
- LFTs
- FBC and erythrocyte sedimentation rate

What is the management plan for diabetic neuropathy?
A
- 1st line Glycaemic control
- w/pain 1st line: Pregabalin or gabapentin and/or duloxetine (neuropathic pain agent)

- Foot wound/ulcers
- Wound infection
- Amputation
- Silent MI
- Death
- Depression
