A
Binds directly to cyanide, forming cyanocobalmin (Vitamin B12) which is readily secreted

A
Known or suspected cyanide poisoning

Contraindications
A
None in emergency setting

Adverse Effects
A
Hypertension, allergic reactions have been reported, N/V, Dyspnea, Indigestion, Dizziness, Bright red skin pigmentation that can last for several days

Precautions
A
Not compatible in same IV with many other medications

Dose
A
5g IV infusion at 15 ml/min over 15 mins.
May repeat x1
Pedi: 70mg/kg at 15 ml/min over 15 mins.
