What is an important state exam tip about transferring a client from a bed to a wheelchair?

What supplies do you need transfer the client from a bed into a wheelchair?

What do you need to do before you begin to transfer the client from the bed to a wheel chair while using a transfer belt?
- State common care beginning script
- Wash your hands
- Put on gloves (in the book it claims the reverse of 1 and 2, call mentor to verify since this contradict infection control regulations)
- Place your supplies on a clean surface
- Ensure the client is in proper body alignment and check their position
- Make sure the wheelchair is prepared and next to the chair or bed.
TIP: The legs of the wheelchair should be off or out of the way of the client, the brakes should be on and the seat belt should be out of the way of the client. The wheelchair should be placed on the care recipient’s unaffected side, next to the bed. Always ask what you are about to do before you do it.
Afterwards, how do you assist the bed ridden client with sitting into the wheelchair if the client can partially assist with a transfer belt?
- Ask to remove their blanket. Fold blanket up, ask to move their feet to the side. Explain what you are about to do (ALWAYS do before you anything) Place one hand on the client’s shoulder (ask if any shoulders are unaffected first, if so place your hand on the unaffected shoulder) and your other hand on the client hip. (ASK) Fold the client’s arm and left leg. (ask mentor why and how they fold their left leg)
- (ASK AND EXPLAIN) Roll them on their side on the last half of the bed. TOWARDS YOU
- On the count of three ask the consumer to push themselves up as you guide their legs off the mattress.
- Make sure the client is a safe distance from the edge of the bed and their feet are flat on the floor (let proctor know they are in the proper body alignment. Help them put on their shoes (look up more info about this.)
- Keep your hands on the client’s shoulder and ask if they feel safe and if they are steady
- Make sure the consumer is wearing non-skid shoes.
What is something that is crucial to remember when moving clients?
You must remember and ask after you move a consumer, “are you confused, dizzy, or in pain or discomfort.
When they are sitting up, how do you apply the transfer belt?
1.Stand in front of the client.
2.Put the transfer belt around the client’s waist area below the ribs. (IMP:Do not put the belt on over the consumer’s head (put on from the side) The belt must always be over clothing, not bare skin. The teeth must be on the outside facing you. (CONTINUE TO PAY ATTENTION TO BODY ALIGNMENT AND PROPER POSITIONING)
3.Put the metal end tip through the teeth of the buckle ( refer to picture beside step 3 in skills manual)
4.Tighten the transfer belt, be careful-but still pull it tight.
TIP:The belt should be snug enough around the consumer so that you should only be able to slip one hand between the consumer and the transfer belt
5.Put the metal tip through the other side of the buckle, adjusting if needed, as shown below in the book via this step.
6.Tuck any excess section of the transfer belt through the waist band of the belt.
How do you them from standing up from the bed with a transfer belt?
- Stand in front of the consumer, feet apart, back straight, with your knees bent, using proper body mechanics.
- Put the leg closest to the wheelchair in front of the consumer’s unaffected leg to brace it. Put your other leg between the client’s leg. ALTERNATE APPROACH:Place your knees on the outside of the client’s legs to brace them.
- Bend your knees so that your shoulders are level with the client.
- Ask the client to put their hands on your shoulders or waist. IMP:Do not let the client put their hands or arms around your neck.
- Place your hands on the transfer belt, as shown below in step illustration-IMP Make sure that your fingers and thumb are pointing up.
- Ask the client to move closer to the edge of their seat.
- Assist consumer to a standing position by using a rocking motion “on the count of three,” as shown in the illustration below in the manual.
How do you transfer partially assist standing client to a wheelchair with a transfer belt?
- Ask how the consumer feels and if they are steady. NOTE Make sure they are in proper body alignment and check their position
- Ask the consumer to take small steps so that they are standing in front of the wheelchair facing you
- Ask the consumer to stand with their back to the wheelchair and move backwards until they are standing centered in front of the wheelchair and can feel the seat of the wheelchair on the back of their legs. IMP Keep your hand on the transfer belt
- Ask the client to reach behind and place their non-affected hand on wheelchair arm.
- Assist the client in slowly easing themselves into the wheelchair “on the count of three.”
- Bend your knees to help the consumer into the wheelchair using proper body mechanics. Go downward with the client as you help ease them into their chair.
- Ask the client to move back on the wheelchair seat.
- Check for proper body alignment and check their position.
Steps for assisting the client to their destination.
1.Ask the client if you can unbuckle and remove the transfer belt.
If yes, then remove the belt and go on to the next steps
If no, simply leave on the belt and go on the next steps.
IMPORTANT Do not pull or drag the belt against the client’s body when you remove it. This could tear the skin.
2.Ask if the client would like the seat belt buckled.
3.(Grab with an opened hand gently near ankle and their foot) Put the footrests down or back on and position the consumer’s feet. (the straps of the footrests go behind the heels)
4.Unlock the wheels and assist consumer to their chosen destination.
5.Ask the client if they want the wheels locked when you get to the destination. Check their body alignment.
When you are finished assisting a client to transfer from bed to a wheelchair with a transfer belt, what do you do?
- Ensure the client is in proper body alignment and check position.
- Put the transfer belt and other supplies away
- Remove your gloves and wash your hands
