Mild/moderate community acquired pneumonia (CURB 0-2).
Amoxicillin (5 days)
Doxycycline if pen allergic
Severe CAP (CURB 3-5)
Co-amoxiclav IV and doxycycline PO
Non-severe hospital acquired pneumonia
Amoxicillin PO
Doxycycline if penicillin allergic
Severe HAP.
IV amoxicillin and gentamicin
Step down co-trimoxazole
Aspiration pneumonia
Non-severe: amox and met
Severe: amox, met and gent
Acute COPD exacerbation
Amoxicillin 1st line
Doxycycline 2nd line
Native valve subacute endocarditis
Amoxicillin and gentamicin IV
Native valve acute endocarditis (severe sepsis)
Flucloxacillin IV
Prosthetic valve or suspected MRSA
Vancomycin and gentamicin IV and rifampicin PO
C. diff infection
Non-severe: PO metronidazole
Severe - PO vancomycin and maybe IV metronidazole
Peritonitis/biliary tract/intra-abdominal sepsis
IV amox, met and gent
Step down co-trimoxazole and metronidazole
If pen allergic: IV vancomycin, metronidazole and gentamicin
Spontaneous bacterial peritonitis
Mild: co-trimoxazole
Severe disease: piperacillin/tazobactam
Complicated UTI/pyelonephritis/urosepsis
IV amoxicillin and gentamicin
Co-trim and gentamicin if pen allergic
Step down is co-trim
7 days
Uncomplicated female lower UTI
Nitrofurantoin or trimethoprim 3 days
Uncatheterised male UTI
Nitrofurantoin or trimethoprim 7 days
