Limitations and caveats to HIGT (high intensity gait training) CPG
A
- Developed for post-stroke patients who had stroke >6 mo back
- Primarily applicable for gait outcomes of ‘speed’ and ‘distance’
- Based on research using individuals who were able to walk without substantial assistance
- May not apply to individuals with limited ambulatory function
- Primarily based on evidence with post-stroke individuals with limited or no direct evidence for iSCI or TBI patients

Clinicians should perform what when using the HIGT CPG
A
- Moderate to high intensity (65-85% HRmax/60-80% HRR) gait training to improve
- gait training using virtual reality to improve walking speed & distance

Clinicians may consider what when using the HIGT CPG
A
- LE strength training (>70% 1 rep max) to improve walking speed and distance
- Cycling or recumbent stepping interventions at higher aerobic intensities (75-85% HRmax)
- Circuit training or combined strategies providing balance, strength, & aerobic exercises to improve walking speed/distance
- Static & dynamic (non-walking) balance strategies coupled with virtual reality or augmented visual feedback to improve walking speed & distance

Clinicians should not perform what when using the HIGT CPG
A
- Sitting/standing balance training directed toward improving postural stability & WBing symmetry b/w limbs or with additional vibratory stimuli
- Body weight supported treadmill training
- Walking interventions with exoskeletal robotics on a treadmill or elliptical devices
