What are right heart problems?
- tricuspid stenosis or regurgitation
- right sided MI
- cardiac tamponade
- ruptured septum
- any condition that affects the RV from ejecting blood into the PA
What are lung problems?
- non-cardiogenic pulmonary edema (aspiration, gram (-) sepsis, ARDS)
- excessive PEEP level
- pulmonary emboli
- alveolar hypoxia leading to COPD
- general pulmonary hypertension
- shunt
What are left heart problems?
- cardiogenic pulmonary edema
- mitral stenosis or regurgitation
- aortic stenosis
- left sided MI
- constrictive pericarditis
- patent ductus arteriosus
- ateriosclerotic heart disease
What is the treatment for patients for landing in subset I (normal, septic shock)?
- treat tachycardia and dysrrhythmia
- nitroglyceride
- beta blockade
- fluid administration
What is the treatment for patients for landing in subset II (fluid overload)?
- IV vasodilators
- ACE inhibitors
- diuretics
What is the treatment for patients for landing in subset III (hypovolemia, dehydration)?
- aggressive fluid therapy
- vasopressors if decreased contractility after volume
- chronotropic drugs with caution
What is the treatment for patients for landing in subset IV (acute left heart failure)?
- vasodilators
- vasopressors
- diuretics
- IABP
- thrombolitic therapy
