Murmur with Aortic Regurg
Decrescendo diastolic
Murmur with Mitral Stenosis
Opening Snap + late diastolic rumble
Murmur with Aortic Stenosis
Ejection Click + Systolic Crescendo/Decrescendo
Murmur with Mitral Valve PRolapse
Midsystolic click + Systolic Crescendo –LOUD OVER APEX
Murmur with Mitral Regurg
Holosystolic - LOUD OVER APEX
Murmur with Tricuspid Regurg
Holosystolic - Loud over Tricuspid
Murmur with VSD
Holosystolic –LOUD OVER TRICUSPID
Murmur with PDA
Continuous Machine-like murmur
Wide Split S2
Pulmonic stenosis or Right bundle block
Fixed Split S2
ASD
Paradoxical Split S2
Aortic Stenosis or Left bundle block
S3 - noise represents what?
Sudden deceleration of blood filling the ventricle as it reaches its elastic limit
S4 - noise represents what?
Rush of blood striking a stiff ventricular wall
S3 seen with what pathologies?
1) CHF 2) MR 3) dilated cardiomyopathy – increased filling pressures
S4 seen with what pathologies?
1) RESTRICTIVE cmpthy 2) HYPERTROPHIC cmpthy 3) LV hypertrophy –Stiff Wall
What does handgrip maneuver make LOUDER?
AR, MVP, MR, VSD
What does handgrip maneuver make QUIETER?
AS, Hypertrophic cardiomyopathy murmurs
What does Valsalva make LOUDER?
AS, MVP
What does Squatting make QUIETER?
MVP, Hypertrophic Cardiomyopathy murmurs
What does Squatting make LOUDER?
MR
What does squatting help with symptomatically?
Tetralogy of Fallot - VSDs are normally L to R but the pulm HTN makes it R-to-L generating a cyanotic tet spell. Squatting increases afterload, and htus LV pressure, reversing shunt.
