How to manage HF, NI?

O2
Monitor for Respiratory distress
To improve SaO2 . To aid in meeting tissue oxygen needs
Raise Head of Bed
If pt is dyspniec, high fowler position with feet horizontal or dangling at bedside helps decrease venous return because of pooling of blood in extremities ; Increased thoracic capacity ; to promote ventilation
IV Access
To administer Meds
Morphine: decreases preload, venous return myocardial O2 , pain and anxiety
-Diuretics: decrease preload / blood volume ex lasix
-Digitalis: Increases cardiac contractility, decreases HR
-Nitroglycerin- Decreases preload by dilating the coronary arteries
Cardiac Monitoring
To access for any cardiac dysrhythmias
Decrease Anxiety
Anxiety increases SNS response and further increases myocardial workload
Maintain activity limitations / rest periods
To reduce myocardial o2 consumption
To conserve O2 demand
Monitor heart/lung sounds
To evaluate response to treatment
Daily weights
To assess any recent weight gain
Monitor I/O
To assess Fluid balance
Apply anti embolism stockings
to help prevent edema , to improve blood flow in legs and to prevent clotting
