How to manage/NI?

Aids in meeting tissue oxygen needs helping to relieve dyspnea and fatigue;;To ensure the adequacy of oxygen supply to the myocardium
Semi Fowlers Position
Promotes Ventilation
Monitor Patient :
BP,Pulse, RR; Cardiac enzymes: Troponin, CKMB, Myoglobin;;lab values
To determine progression of MI;To assess for Myocardial Cell death / reversable ischemia; To evaluate treatments and detect development of hr and cardiogenic shock; assess Q15m until stable then Q1hr
Maintain/ obtain IV Acces
To administer medications per dr orders
morphine = decreases preload, sns response, myocardial o2 consumption , pain and anxiety
nitroglycerin: decreases preload and after load by dilating coronary artery ; improves collateral flow to ischemic regions; relieves pain
aspirin: inhibits platelet aggregation
Monitor ECG/ Cardiac monitoring
To determine any cardiac changes ; dysrhythmias ; HR
Assist patient with ADLs/ bedrest/rest periods
To conserve energy and reduce oxygen demand
Assess peripheral vascular Status/ Monitor I/O
To determine if MI is progressing/ has progressed to Heart Failure
Anticipate the need of thrombolytic
Given :
ideal within 1 hr
or within first 6 hours following MI
Aids in Dissolving clot
Anticipate PCI/CABG surgery
To establish blood flow balloon / stent. Redirect blood flow distal to blockage ; Internal mammary Artery , saphenous Vein
