How to manage/NI?

Tissue oxygen needs
Monitor BP/Pulse/RR
BP may be increased-hypervolemia during oliguric phase
BP may be decreased-hypovolemia during diuretic phase
Monitor I/O - strict I/O
Fluid balance / fluid retention
Obtain Daily weights
determines fluid retention
1kg increase= 1 L fluid retention
Monitor for electrolyte imblances esp Potassium
at risk for cardiac dysrhthmias
Review patients med regimen to look for nephrotoxic med/ Discontinue nephrotoxic med
to prevent further toxicity to kidneys
Maintain aseptic technique
infection is leading cause of death in ARF
Monitor BUN/Creatinine levels
Increased due to failing kidney(s)
bedrest
to decrease metabolic rate
Meticulous skin care
patients usually develop edema and decreased muscular tone ; prevent pressure ulcers
prepare patient for dialysis
failing kidnery - aids in removing harmful waste, and excess water from blood
WHAT TO REPORT TO PHYSICIAN?
Urine output!
