Why is sacubatril and valsartan combined in the treatment of HF?

A

Need to block the effects of RAAS which cancels out the natriuretic peptide system (increased natriuresis and diuresis) which is what ARNI’s are acting on. Therefore an ARB is combined to block the effects of AT1 and AT2 (stopping a decrease in natriuresis and diuresis).

What is Digoxin’s MOA?

A

Inhibits Na+K+ ATPase pump on the heart cells. This causes an increase in Na+ intracellularly (heart cells) which increases intracellular Ca2+ which increases force of contraction.
