Which organisms are associated with antibiotic associated diarrhoea?
C. Diff.
Summarise the epidemiology of HAIs.
- 4 million people in Europe get a HAI every year
- ~ 37,000 die as a direct result of the infection
- Antimicrobial use - key driver of antimicrobial resistance (AMR)
7.8% of patients in Europe
What is the most common HAI?
Hospital acquired pneumonia
What are other common causes of HAI?
Surgical site infection - 19%
Urinary tract infection - 17%
Bloodstream infection - 14%
Gastrointestinal infection - 8%
What is C. Difficile?
Gram positive spore forming anaerobe. Spores are transmissible, contaminate environment, persist for long periods. Ingested spores germinate in gut. Gut flora disturbed by abx exposure, to different extents.
C. difficile produces toxins A and B.
- Diarrhoea, colitis
- Toxinotype III / BI / NAP1 / 027
- Pro-inflammatory – causes inflammation
- Cytotoxic – cell damage
- Enterotoxic – damage to the gut
What is a good antibiotic for C. Difficile?
Fidaxomicin - macrolide antibiotic
What are non-drug approaches to treating C. Difficile?
Faecal transfer: Fresh faeces, healthy donor, in saline, filtered
What percentage of catheters become colonised?
60% colonised within 96h
