What is the purpose of the “Lock Out / Tag Out” (LOTO) procedure?

A

To ensure equipment is shut off and has no possible way to be re-started when work is being carried out

Briefly describe the procedure of “Lock Out / Tag Out” (LOTO)…

A

Identification of hazardous sources or dangers within the work environment
Those dangers are isolated
Isolations are locked with padlocks and tags put on to indicate they are shut off
Further process can be to “Try Out” - trying to operate the equipment to ensure that its not working

Explain the process of conducting an enclosed space entry in general?

A
  1. Consult risk assesment and identify any additional hazards and their associeated risks
  2. put in place the control measures as written in risk assesment and permit to work
  3. Ensure rescue gear is in place and open tank
