Pain Sensitive Structures in the Brain
Dural sinus, venous sinus, intracranial portion of V, IX, X, upper cervical nn, large arteries
Factors that affect pain sensitive structures
Tension and traction, vascular throbbing, inflammation
Pain Insensitive structures
Brain parenchyma (intracerebral lesions), ependymal lining of ventricles, choroid plexus
Migraine
Common Migraine: w/o aura, lasts 4-72h, unilateral, pulsating, moderate severity, physical activity worsens it, vomiting, nausea, photophobia
Classic Migraine: w aura, focal neurologic deficit, visual disturbances
Due to neurovascular dysfunction
Tension-type/Muscle contraction headache
diffuse, mild to moderate pain in your head that’s often described as feeling like a tight band around your head, bilateral squeezing
Cluster Headache
cyclical patterns, painful, pain around one eye or side of head, alcohol exacerbates
Secondary headache
arises secondary to a known cause: CVA, Inflammation, infection
Headache RED FLAGS
Age (>50), no prior history or change in headaches, papilledema (swelling of optic disc due to intracranial pressure), thunderclap headache (severe, sudden), systemic illnesses, neurological symptoms
Medication Overuse Headache
Headache >15d/mo; Medication use >10d/mo; headache from overuse
