How do you treat Shock?
fluid challenge 300-500ml over 10-20 mins + monitor response
What possible fluids can be used for fluid challenge?
crystalloids
colloids
blood
What should be used if fluids don’t work in a fluid challenge?
noradrenaline - alpha agonist causing vasoconstriction
dopamine
What is class 1 hypovolaemia?
<15% blood loss, monitor
What is class 2 hypovolaemia?
15-30% blood loss, possible need for blood products, decreased pulse pressure, increased HR
What is class 3 hypovolaemia?
31-40% blood loss, need for blood products
decreased pulse pressure, BP, GCS, urine output
increased HR and RR
What is classi 4 hypovolaemia?
> 40% blood loss, need for massive transfusion protocol
decreased pulse pressure, BP, GCS, urine output
increased HR and RR
What blood figures do you want to maintain during haemorrhage?
Hb >80
APTT + PT <1.5
Platelet >50
Fibrinogen >1.5
