Normocytic, normochromic anaemia
(Microcytic in some cases)
Pathophysiology - Anaemia of Chronic Disease
Continuous inflammation due to chronic disease causes impaired iron metabolism = reduced RBC production
Presentation - Anaemia of Chronic Disease
Incidental finding
Symptoms of anaemia = SOB, fatigue, pallor
Iron Studies Results - Anaemia of Chronic Disease
Serum iron = low
TIBC = low
Transferrin saturation = low
Ferritin = high
Management - Anaemia of Chronic Disease
Treat underlying cause
Management (Unresponsive/Severe Cases) - Anaemia of Chronic Disease
RBC transfusions
Erythropoesis stimulating agents
Supplemental iron
Causes - B12 Deficiency
Absence of dietary B12 (vegan diet)
Drugs e.g. PPIs
Pernicious anaemia
Presentation - B12 Deficiency
Diarrhoea
Glossitis
Paraesthesias
Anaemia
Weight loss
Type of Anaemia - B12 Deficiency
Megaloblastic anaemia - a type of macrocytosis
Blood Results - B12 Deficiency (Anaemia)
FBC = high MCV, low Hb
Serum B12 = low
Blood film = hyperhsegmented neutrophils
Management (Symptomatic) - B12 Deficiency
1mg IM tydroxycobalamin 3 times weekly for two weeks, then once every 3 months
Management (Asymptomatic) - B12 Deficiency
Dietary supplementation + multi-vitamins
Oral cyanocobalamin
Complications - B12 Deficiency
Subacute combined degeneration of the cord
Daily requirement of B12 and why?
1.5ug/day
Needed for developmentt of RBCs
Site of B12 absorption
Ileum
