Mechanism of Action (MOA)?
A
Dextrose 50% MOA:
- Reverses CNS effects of hypoglycemia by rapidly increasing serum glucose levels.

Indications?
A
Dextrose 50% Indications:
- Hypoglycemia
- ALOC of unknown etiology
- Seizures of unknown etiology
- Hyperkalemia

Contraindications?
A
Dextrose 50% Contraindications:
- Known thiamine deficiency (relative, if suspected give thiamine)
- Delirium tremens
- Head injury (unless documented hypoglycemia)
- Intercranial hemorrhage (relative)

Adverse Reactions?
A
Dextrose 50% Adverse Reactions:
- Cerebral edema in children when given IV undiluted
- Worsening elevated ICP
- Extravasation leads to severe tissue necrosis

Adult Dose?

Dextrose 50% Adult Dose
- Hypoglycemia, ALOC or SZ of unknown etiology: 25-100 ml of D50 (12.5 - 50 Gm) IV.
- Hyperkalemia: 50 Gm of D50 IV administered with calcium chloride and sodium bicarbonate.

Pediatric Dose?
A
Dextrose 50% Pediatric Dose:
- 0.5 - 1 Gm/kg of dextrose 10% solution; recommended slowly over 20 minutes.
- To prepare dextrose 10% , waste 50 ml of normal saline from a 250 ml normal saline “bag” and add 50 ml of dextrose 50%.
