Diltiazem (Cardizem)
Mechanism of Action (MOA):
- Decreases SA and AV conduction
- Dilates coronary and peripheral arteries/arterioles
Diltiazem (Cardizem)
Indications?
Diltiazem (Cardizem) Indications:
- Rapid ventricular rates associated with atrial fibrillation and atrial flutter
- SVT refractory to adenosine
Diltiazem (Cardizem)
Contraindications?
Diltiazem (Cardizem) Contraindications:
- Hypotension (less than 90 mmHg systolic)
- Acute MI
- Cardiogenic Shock
- Wide complex tachycardia, VT
- Wolff-Parkinson-White (WPW) syndrome
- Sick Sinus Syndrome

Adverse Reactions?
Diltiazem (Cardizem) Adverse Reactions:
- CV: hypotension, bradycardia, heart block, chest pain, asystole
- GI: nausea and vomiting
- CNS: headache, fatigue, drowsiness
Diltiazem (Cardizem)
Adult Dose?
Diltiazem (Cardizem) Adult Dose:
- Initial: 0.25 mg/kg IVP (usually 20 mg) administered over 2 minutes
- Repeat: If response inadequate, repeat in 15 minutes with 0.35 mg/kg IVP administered over 2 minutes
- Maintenance Infusion: 5 - 15 mg/hr
