Root Cause Analysis
to identify root causes to a problem is the key of the analyze phase
The Cause and Effect, or Fishbone, Diagram
Fishbone diagram is a cause and effect diagram with the effect at the head of the fish/problem and the effects as the scales of the fish.
Root Cause Verification Matrix
You have the table:
problem, problem root causes, method of verification, reason for verification, verified (y/n)
Graphical Analysis
Graphical analysis is a way to graphically represent data about those root causes that have been recorded.
Statistical Analysis
Hypothesis Testing
Alpha Risks/Beta Risks
p-value
correlation/regression analysis
Analyze Tollgate Checklist
Before you leave the Analyze phase, please look at the checklist:
- ) Your primary root causes have been identified.
- ) Team has prioritized the root causes
- ) Champion/Sponsor seconds the team to move on to “Improve Phase”
- ) Relationships between variables are understood between causation
- ) These variable causes have been verified by statistical analysis.
